REGULATIONS FOR PARTICIPATION IN FAMILY SCHOOLS and WORKSHOPS at KOALA Family Maternity Center

§1 GENERAL PROVISIONS

1. Classes of the Childbirth School and Workshops are held in the rooms of the KOALA Family Maternity Center located at 1 Rhetoric Street, premises 18 and 17, in Cracow.

2. Between 4 and 8 couples participate in Birth School classes. In special cases, it is possible to increase the number of people.

§2 ACCESS TO AND USE OF SERVICES

1. Applications to participate in Birth School classes and Workshops are made through the registration form available on the Birth School website: https://www.koala.krakow.pl/

2. To apply for a Birth School class and Workshop, select a date for the Birth School course or Workshop from those available in the Schedule, and then the interested person fills in all fields of the registration form and sends the application to the Organizer by clicking on the “sign up” button visible under the form.

(4) After sending the application, the Organizer shall confirm to the applicant (who shall hereinafter be referred to as the “Participant”) the acceptance of his/her application and the reservation of a place at the selected classes of the Childbirth School or the Workshop by sending the appropriate confirmation to the e-mail address provided in the registration form within 24 business hours from the sending of the application.

§3 FEES FOR BIRTHING SCHOOL CLASSES

1. Participation in the classes of the Childbirth School is subject to a fee. The amount of the fee depends on the type of classes chosen by the applying Participant. (+48) 692 992 555, e-mail: koala@koala.krakow.pl The regulations apply to admission to birthing school classes and use of services offered by Anna Przybylska Rodzinne Centrum Obstetric Koala. 10c/3 Nuremberg Street 30-376 Kraków. Place of business is located at 1/18 Rhetoric Street in Krakow (31-108 Krakow), NIP: 6762086747, REGON: 120278640. Each Participant of the Childbirth School, Workshops offered at RCP Koala is obliged to familiarize himself/herself with these regulations of RCP Koala and compliance with its provisions. in the classes of the Childbirth School are available on the website of the Childbirth School: https://www.koala.krakow.pl/cennik/. The applying participant(s) is bound by the fee in the amount listed on the aforementioned website at the time the Participant sends her/his application for the class.

2. The fee for participation in the classes of the Childbirth School includes the participation of a pregnant mother (Participant) and an accompanying person of her choice, who is not a pregnant woman. The fee does not change if only one person participates in all or part of the classes of the Childbirth School.

3. The fee for participation in the classes of the Childbirth School includes a full package of theoretical and practical classes (lectures, workshops and exercises) offered by the Organizer, within the framework of the Childbirth School course selected by the Participant. The fee does not change if the Participant or her companion takes part or intends to take part only in part of the classes offered as part of the package.

4. The participant(s) who signed up for the classes of the Childbirth School via the registration form shall receive information from the Organizer on the amount of the fee payable by him/her to the e-mail address provided by him/her. Such information is provided along with confirmation of booking a place in the classes.

6. The fee for the classes of the Childbirth School may be paid in cash, by card or by blister at the cash desk located at the Organizer’s headquarters or by bank transfer, to the bank account of the Organizer indicated in the e-mail containing information on the amount of the fee due for the classes of the Childbirth School.

7. The amount due fromParticipant( for the Birth School course is divided into two payments:

– The first payment of PLN 400 is treated as a deposit and is non-refundable in the event of cancellation of the course, it should be paid within 4 days from receipt of booking confirmation;

– The second part of the fee of the remaining amount (the full amount of the course minus the deposit) must be paid in the first week of the course.

8. The participant/s also has the right to pay the entire fee due at one time. In this case the deposit is still valid. Therefore, in the event of cancellation of the course, the Organizer will refund the Participant the fee minus the amount of the deposit.

9. In the case of selection of the Childbirth School course as a package with other services, a one-time payment of the total amount according to the price list at: https://www.koala.krakow.pl/cennik/, within 4 days of receiving confirmation of the reservation, of which PLN 400 is treated as a deposit and is non-refundable.

10. In case of non-payment of the deposit due from the Participant(s) on the date indicated above, the Organizer is entitled to cancel the Koala Family Maternity Center, tel. (+48) 692 992 555, e-mail: koala@koala.krakow.pl reservation of the Participant(s) for her selected classes of the Childbirth School, without the need to separately notify the Participant of the cancellation.

11. The organizer or entities affiliated with the organizer may introduce the possibility for persons interested in enrolling in the classes of the Childbirth School to obtain discount coupons. The participant(s) applying for the classes of the Childbirth School shall provide information about the discount coupon held in the registration form in the field “additional information” or in response to the e-mail confirming the enrollment. A participant who does not inform the Organizer about her/his discount coupon in this manner shall bear full payment for the course of the Childbirth School.

§4 CANCELLATION OF PARTICIPATION IN BIRTHING SCHOOL CLASSES

1. A participant enrolling in the classes of the Childbirth School may cancel her participation at any time.

2. The Participant shall inform the office of the Childbirth School about resignation from participation in the classes of the Childbirth School by sending appropriate information about the resignation to the e-mail address of the School’s coordinator: koala@koala.krakow.pl . The resignation does not require justification – subject to the provisions of § 6 paragraph 4 below.

3. The booking fee for the Childbirth School course in the amount of PLN 400 is non-refundable.

4. A participant who, for medical reasons, resigned from participation in the classes of the Childbirth School after payment of the full fee due from her for the classes of the Childbirth School, but before the realization (completion) of the entire Package of Classes assigned to her, she may apply to the Organizer to transfer part of the funds for the unrealized classes to other workshops, courses, services, available within the activity of the Organizer. In such case, the Participant is obliged to submit a request to the email address koala@koala.krakow.pl, stating the reason justifying her resignation from the classes of the Childbirth School.

§5 WORKSHOP FEES

1. The Organizer offers Participants and their accompanying persons the opportunity to take part in Workshops conducted outside the block of basic classes of the Childbirth School.

3. Participation in the Workshop is subject to a fee. The amount of the fee depends on the type of classes chosen by the applying Participant. Information: Koala Family Maternity Center, tel. (+48) 692 992 555, e-mail: koala@koala.krakow.pl about the current Workshop fees are available on the website: https://www.koala.krakow.pl/cennik/. The applying Participant(s) is bound by the fee in the amount listed on the above-mentioned website at the time the Participant(s) sends her/his application for the class.

4. The fee for the classes of the Childbirth School may be paid in cash, by card or by blister at the cash desk located at the Organizer’s headquarters or by bank transfer, to the bank account of the Organizer indicated in the e-mail containing information on the amount of the fee due for the classes of the Childbirth School.

§6 CANCELLATION OF PARTICIPATION IN THE WORKSHOP

1. Rules for cancellation of participation:

– Cancellation up to 4 working days before the workshop – 100% refund of the deposit

– cancellation 3 working days prior to the workshop – the possibility of transferring 50% of the payment to another selected workshop date.

– attempted cancellation on weekends and holidays (Friday after 4 p.m. to Monday 9 a.m.) – no refunds

§7 FINAL PROVISIONS

1. If changes are made to the content of the Rules and Regulations, the changes shall become effective as of the date of their publication on the website of the Childbirth School https://www.koala.krakow.pl/. The Rules and Regulations as in effect on the date of application for participation in the classes of the Childbirth School and/or the Workshops prior to the effective date of the amendments to the Rules and Regulations shall apply to the Participants who applied for participation in the classes of the Childbirth School and/or the Workshops.

2. These Regulations are available to the Participants, and their accompanying persons, and all interested parties on the website of the Family School https://www.koala.krakow.pl/ and are posted at the Koala Family Maternity Center Headquarters.

3. The regulations come into force on July 01, 2024.
