REGULATIONS FOR PARTICIPATION IN GYMNASTICS CLASSES at the KOALA Family Maternity Center

Regulations apply to admission to gymnastics classes and use of services offered by the company Anna Przybylska Rodzinne Centrum Położnicze Koala 10c/3 Nuremberg Street 30-376 Kraków. The place of business is located at 1/16 Retoryka Street in Krakow (31-108 Krakow), NIP: 6762086747, REGON: 120278640. Each Participant of gymnastics classes at RCP Koala is obliged to read and abide by these rules and regulations of RCP Koala.

GENERAL PROVISIONS

1. Gymnastics classes, i.e. breathing in labor, gymnastics with the tummy, pelvic floor muscle training, Pilates with the tummy, yoga with the tummy, stretching with relaxation, healthy spine with the tummy, gymnastics on balls, gymnastics with the toddler, yoga with the toddler are held in the rooms of the KOALA Family Maternity Center located at 1/16 Rhetoric Street in Krakow.

2. Classes are conducted by qualified staff, specialists in physiotherapy, instructors of physical recreation.

3. Pregnant women’s classes are attended by 3 to 15 ladies, and postpartum moms’ classes by 3 to 10 ladies. In special cases, it is possible to increase the number of people.

ACCESS TO AND USE OF SERVICES

1. Registration for classes is conducted through the Fitssey application https://app.fitssey.com/rcpkoala. The Fitssey class booking system is operated by the company lightenbody, headquartered at 241/13 Zwycięstwa Avenue, 81-521 Gdynia, NIP: 7532294721, REGON: 362814110, to which Koala Family Maternity Center has entrusted the processing of personal data.

2. The implementation of the sale of passes requires the processing of personal data of the Participant, who undertakes to provide true and current data. By registering, the Participant agrees that RCP Koala may store and process his or her personal data in order to properly perform services and ensure efficient contact between RCP Koala and the Participant.

3. The Participant’s personal information is also used to provide information about RCP Koala’s current offerings, including ongoing workshops and courses only with the consent of the person providing their personal information.

4. The storage and processing of the Participants’ personal data shall be carried out in compliance with the principles set forth in the Regulation of the European Parliament and of the Council of the EU 2016/679 (dated April 27, 2016, Official Journal of the EU.L. No. 119), in the Polish regulations in the scope of RODO, and in the Act of July 18, 2002 on the provision of electronic services (Official Journal of 2020, item 344, as amended). Any person who provides his/her personal data has the right to access it, as well as to correct, update and delete it.

BOOKING AND CANCELLING CLASSES

1. Classes are held according to the schedule, posted on www.koala.krakow.pl and available in the analogue version at each class, provided that class groups are gathered. In case of insufficient number of willing participants, RCP Koala reserves the right to cancel the class no later than 4 hours before the start of the class. Participants will be notified by text message or telephone.

2. Prior to the first class, the participant should create an account on the https://app.fitssey.com/rcpkoala/frontoffice application

3. It is possible to book a place in the classes only through the Internet via the sites:

4. Enrollment is accepted at the earliest 14 days before the class for participants who have the RCP Koala pass and students of the RCP Koala Birth School. For those with Multisport and Fitprofit cards, as well as Medicover sports, enrollment is accepted 7 days before the class.

5. One entry is charged for each unreported absence or late cancellation from the pass. Absence or late cancellation is understood as failure to cancel the class in accordance with the applicable cancellation policy.

6. Participants using Multisport, Fitprofit, Medicover partner cards to activate the ability to book and cancel classes must pay a deposit in the Koala Family Maternity Center, tel. (+48) 692 992 555, e-mail: koala@koala.krakow.pl amount of PLN 40 through the Fitssey application. The unused deposit can be returned to the customer upon request.

7. Participants using Multisport, Fitprofit, Medicover partner cards will be automatically charged PLN 20 for each unreported absence or late cancellation from the virtual deposit wallet. Absence or late cancellation is understood as failure to cancel classes in accordance with the applicable cancellation policy.

8. The participant has the option of cancelling his or her enrollment in the class free of charge via fitssey app, SMS or phone contact 4 hours before the class

9. In special cases, it is possible to extend the pass, provided that you report your absence in advance.

10. Participants on the reserve list will be informed of the opportunity to take the class no later than 15 minutes before the start of the class. Participants themselves determine when reserving a place on the reserve list how long they will wait to be included in the list of participants. Confirmation of enrollment will be made by email.

ATTENDANCE

1. Ladies from 16 weeks of physiological pregnancy are welcome to attend classes, after consultation with the attending physician (we do not require written certificates).

2. In order to participate in the classes before 16 weeks of pregnancy or in the case of multiple pregnancies (twins), you must provide before the first meeting a doctor’s certificate that there are no contraindications to participation in classes for pregnant women.

3. Due to the nature of the exercises, ladies who experience: spotting, bleeding and other ailments during pregnancy cannot participate in the classes.

5. Before the class, you must present your pass, Silver or Gold Card, Multisport/Fitprofit card or pay a one-time fee.

6. Multisport and Fitprofit cards, as well as Medicover sport cards are accepted only upon presentation of proof of identity.

7. During the class, ladies are required to:

● each time to inform the instructor about worse mood or ailments during exercises,

● bring sportswear and appropriate and clean shoes for a change; it is also possible to practice barefoot,

● To observe order and maintain cleanliness.

8. The participant is required to attend class on time. Acceptable lateness is 10 minutes. After this time, the participant will not be allowed into the class and his/her tardiness will be marked as an absence, with all consequences.

PAYMENT, RETURNS AND COMPLAINTS POLICY

1. The passes are valid for a period of 30 days, starting from the first participation in the class.

2. Payment for passes (according to the current price list) can be made in cash, by card before or after the class, and by bank transfer. In case of payment by wire transfer, please send confirmation of payment to koala@koala.krakow.pl before the class. Price list:

● single entry: 40 PLN

● 4 entry pass: 140 PLN

● 8-entry pass: PLN 200

● OPEN pass: 300 PLN

● Golden Card: 2000 PLN (entrances to any classes for 4 months of pregnancy, Childbirth School)

● Refundable deposit for partner cards: Multisport, Fitprofit, Medicover: 40 PLN

3. Fees for unused classes are not refundable.

4. In justified cases, the fee for a fully unused pass is refundable within 30 days.

5. Complaints are considered individually within 14 working days.

4. Promotions and discounts do not combine.

5. Bills are issued only on the basis of the presented fiscal receipt on the day of issue.
